Consent Form for Body Piercing and Tattoo Services
AidanKai LLC DBA TNT Tattoos

Age Confirmation:
I, the undersigned, affirm that I am at least 18 years of age. If I am a minor, this form is signed and dated by my parent or legal guardian.

Understanding of Tattoo Permanence:
I acknowledge that tattoos are permanent and can only be removed through surgical procedures, which may result in permanent scarring.

Health and Medical History:
I confirm that I do not have a heart condition, epilepsy, hepatitis within the last year, hemophilia, HIV, AIDS, or any strain of Covid-19. I am not currently pregnant or breastfeeding. I have not had jaundice within the past 12 months. To my knowledge, I do not have any physical, mental, or medical impairments or disabilities that may affect my well-being as a result of tattoo or body piercing work.

Risk Acknowledgment for Body Piercing:
I acknowledge that body piercing carries risks, including the potential for allergic reactions and infections. I understand these risks and choose to proceed with the body piercing.

Responsibility for Aftercare:
I agree to follow all instructions for the care of my tattoo or body piercing during the healing process. I acknowledge that any touch-up work needed due to my negligence will be at my own expense.

Tattoo Appearance on Dark or Tanned Skin:
I understand that if my skin is dark or tanned, the colors of the tattoo may not appear as bright as on light skin.

Sobriety Confirmation:
I confirm that I am not under the influence of drugs or alcohol.

Photo Consent:
I consent to the use of photos of my tattoo or body piercing for marketing purposes by AidanKai LLC DBA TNT Tattoos.

Release of Responsibility:
I release AidanKai LLC DBA TNT Tattoos and its representatives from all responsibility and accept any consequences arising from my decision to have tattoo or body piercing work done.

Hold Harmless Agreement:
I agree for myself, my heirs, assigns, and legal representatives to hold AidanKai LLC DBA TNT Tattoos harmless from all damages, claims, demands, rights, and causes of action arising from my decision to have tattoo or body piercing work done.

Premises Departure Agreement:
I agree to leave the premises of AidanKai LLC DBA TNT Tattoos promptly upon request, for any reason, by any agent or employee.

Scope of Waivers:
These waivers also extend to protect any and all establishments where AidanKai LLC DBA TNT Tattoos or any other AidanKai LLC DBA TNT Tattoos employee performs services.
